New Patient Registration
The doctors welcome new patients who live within our practice area. Click here to see if you are within our catchment area.
It often takes sometime for records to be forwarded from your former practice.
You will be asked to have proof of ID and address and to complete a new patient registration pack before you can be offered a consultation with the doctor. A new patient medical can be arranged with our Health Care Assistant if you would like one.
New Patient registration process
Step 1:
If you could come in to the Practice and ask to register. The receptionist will ask you to fill in a white form and then check to see if you are within our practice area. We then ask if you could ring back after 2 days to see if you have been accepted at our practice.
Step 2:
The receptionist will give you a new patient registration pack if you have been accepted. Each page needs to be completed and handed back into the Practice. Please bring a photo ID and proof of address from the table below:

Please note: It is essential you fully complete the new patient pack and hand it back into the surgery. We would not turn people away if it was an emergency.
